    Default Name Change

    Being a woman, naturally when I married I took my husbands name. We are headed to divorce court, when and how do I get my maiden name back?

    At final hearing. You would request the final order contains language giving you a name change.

    If you are the Petitioner, you can include the request for a name change in your Petition for Dissolution.
