Present Monthly Deductions

What do they want on this form? Do I add up all my pay stubs for the month, and put in the amount they deducted for all the gov,state,and fed taxes? On the alimony part do they want the amount he pays, or the true amount I actually get after SDU takes out their fees? Please answer these questions,this is all I need to finish my paper work,I really don't know what they want.....HELP PLEASE!!!!!!!!!!!
